How to unlock all characters, completion marks, and hidden content in The Binding of Isaac: Rebirth.

Character Unlocks

MagdaleneHave 7 or more Red Heart containers at once.
CainHold 55 or more coins at once.
JudasDefeat Satan (boss of Sheol).
EveDon't pick up any hearts for two consecutive floors.
SamsonComplete two floors in a row without taking damage.
AzazelMake three deals with the Devil in one run.
LazarusHave 4 or more Soul Hearts or Black Hearts at once.
EdenDefeat Mom's Heart (unlocked from the start in Repentance).
The LostDie in a Sacrifice Room while holding Missing Poster.
LilithDefeat Greed Mode for the first time.
KeeperDonate 1000 coins to the Greed Donation Machine.
ApollyonDefeat Mega Satan for the first time.
The ForgottenStart a run on the first floor, bomb the starting room to find Mom's Shovel piece, then complete the quest.

Completion Marks

Mom's Heart

Defeat Mom's Heart in the Womb. Required to unlock further paths.

Isaac

Defeat Isaac in the Cathedral. Unlocks character-specific items.

Satan

Defeat Satan in Sheol. Unlocks character-specific items.

Blue Baby

Defeat ??? (Blue Baby) in The Chest. A major completion milestone.

The Lamb

Defeat The Lamb in the Dark Room. Unlocks character-specific items.

Mega Satan

Defeat Mega Satan using both Key Pieces from Angel Rooms.

Hush

Defeat Hush by reaching the Blue Womb within 30 minutes.

Delirium

Defeat Delirium in the Void. One of the hardest bosses in the game.

Greedier

Complete Greedier Mode (harder version of Greed Mode).

Mother

Defeat Mother in Corpse II via the Repentance alt-path.

The Beast

Defeat The Beast at the end of the Ascent in Repentance.
